The following clause was created on the basis of art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27/04/2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as GDPR). It is used when the data has been obtained directly from the data subject.

1. Data administrator

The administrator of personal data is Calisia University – Kalisz, Poland with its registered office at Nowy Świat 4, 62-800 Kalisz.

2. Data Protection Officer

The administrator has appointed a Data Protection Officer, who can be contacted via email: iod@uniwersytetkaliski.edu.pl or by phone +48 62 76 79 690.

3. Processing rights

You have the right to access the content of the data, rectify it, limit processing. Within your consent to the processing of personal data, you can withdraw it at any time, which will not affect the lawfulness of the processing that was carried out before its withdrawal.

You have the right to object to the processing based on art. 6 sec. 1 lit. e) GDPR, for reasons related to your particular situation.

4. Information about the right to lodge a complaint

You have the right to lodge a complaint with the President of the Office for Personal Data Protection if you believe that the processing of your personal data violates the GDPR.

5. Purpose and legal basis for processing

The personal data provided by you will be processed for the purposes of:

a) registering participation in the 4th Calisia World Conference on Family Health, communicating with participants in order to organize a scientific event, document its course and to send information about scientific events by Calisia University – Kalisz, Poland in the future.

The legal basis is Art. 6 sec. 1 lit. e) GDPR, in connection with the Act of 20 July 2018 Law on Higher Education and Science (Journal of Laws of 2023, item 742) and the mission carried out by the university, which is conducting scientific activity, shaping civic foundations and participation in social development;

b) promote the scientific event, in particular in information and promotional materials. With regard to the image – recorded during the meeting in the form of photographs, video recordings from the course of the meeting, the processing takes place on the basis of art. 6 sec. 1 lit. a) GDPR, in connection with the consent granted pursuant to art. 81 sec. 1 of the Act of February 4, 1994 on copyright and related rights, to the extent and time indicated in the participant’s declaration of consent to the recording and dissemination of the image.

6. Obligation to provide personal data

Providing personal data is voluntary, however, failure to provide this data will prevent you from participating in the 4th Calisia World Conference on Family Health.

7. Processing time

The personal data provided by you and processed in connection with your participation in the scientific event will be processed for the period of storage by the university with regard to financial, accounting and reporting documentation connected with this event.

8. Recipients of personal data

Your personal data will not, as a rule, be disclosed to other entities, except for entities authorized under the law. If the administrator uses the services of other entities, personal data may be disclosed to them on the basis of personal data processing agreements, and these entities will be obliged to maintain the confidentiality of the processed data.

9. Information about not making automated decisions

The personal data provided by you will not be profiled and no decisions will be made in an automated manner based on this data.
